FSN Accepting Nominations for the FSN Lifetime Achievement Award
All applications must be received by the FSN no later than October 1, 2019.
The FSN Lifetime Achievement Award was created as our organizations highest recognition to be given periodically to individuals who have, during the course of their career, demonstrated either outstanding leadership in organized medicine, or made significant contributions to the practice of nephrology in the State of Florida or to organizations important to nephrology in the State (i.e. FSN). Additionally, those whose impact was in either creating novel or creative educational programs at our teaching centers or meaningful contributions to research in Nephrology.
Given this award is the highest honor our organization will bestow on an individual, the organization does not intend to award an honoree each year.
Procedure For Nominating And Criteria For Selection Of Recipients Of The Lifetime Achievement Award.
A request to nominate a candidate must be submitted to the FSN accompanied by:
2) A nomination letter that includes, as appropriate, a detailed description of the candidates participation in FSN programs, their practice of nephrology and the impact they had in the community, research contributions made that contributed to advances in the diagnosis and/or treatment of kidney disease, involvement in educational programs at the teaching programs within the State and important community-based activities in the State of Florida.
